Talent Release Form

I hereby give permission to destinationTV.com (herein referred to as “Company”), it's owners and operators, affiliated broadcast television stations, cable stations, online streaming networks and social media sites working with Company to use my name, photo, and video likeness in all marketing and media for electronic and printed productions, advertising, promotional purposes, and in conjunction with affiliated companies and any other lawful purposes. Company owns and is using likeness for the purpose of public mainstream production. I understand that if my likeness is published it can be used in perpetuity and any removal requests can be denied or if possible, performed at an additional fee.

Company does compensate for selected footage and where monetary compensation my be offered upon public publishing. In some case compensation may be performance based on ratings or views over time. Email correspondences between the parties shall establish such agreements where responses between the parties are clear and mutually stated as understood. I further understand, that there is no warranty of payment, exposure or royalties unless expressly agreed to in writing.

This talent release agreement supersedes all other agreements or may work in conjunction with other agreed to terms and where other agreements are in writing and consist of talent release language.

I have read this release, understand the scope of the productions and upon submission of this form approve of its terms.
